1. All forms are in the finance office or D22. Please use updated forms.
2. Only forms filled out properly, with signatures from certificated staff members, will be accepted for approval.
3. Activity Request Forms for fundraisers and activities must be submitted 3 weeks in advanced with a proposal (see JRod for it).
4. If meeting minutes are not attached with activity requests and PO's, the event or purchase will not be approved.
5. Fundraisers may not take place without prior approval
